They aim to demonstrate that special meals and education provided to patients with congestive heart failure can improve patient outcomes and reduce hospitalizations and health care costs.
The local partners for this statewide program are Ceres Community Project and Food for Thought. KRCB news director talked with Food for Thought executive director Ron Karp about the program.
